First call:

From code: GET /api/v1/meta/usage/ with your X-Api-Key header returns the same numbers as JSON.

Every request to a data endpoint under /api/v1/ made with your key counts, including ones that return 400 for a bad query.
Requests refused with 403 (bad key) or 429 (over a limit) do not count. Calls to /api/v1/meta/ do not count.
The month is the UTC calendar month. Over the monthly allowance the API answers 429 with a Retry-After header until the first of the next month.
Over 45 requests in a minute the API answers 429 for the rest of that minute, as PatentsView did.
